We strive to treat your clients as you would want them treated. Our goal is to work in conjunction with you as a veterinary care professional by extending the services that you provide to your clients.
We are pleased to accept your referrals. Please complete a Referral Form, available as a PDF or Microsoft Word document. You may

Prior to your client's appointment at our clinic, please fax the patient's recent medical records, including surgery reports and lab tests. Please have your client bring their most recent imaging studies as well. If, during the course of our treatment, you are to perform additional imaging studies, please also send those with the client for our viewing information. Finally, please alert us to any existing medical or surgical conditions, including current medications.
At your client’s initial appointment, we will structure an in-clinic therapy plan for the patient. Additionally, a home exercise plan will be issued which will be updated as the patient progresses through therapy. A written summary of their visit will be mailed or faxed to you so that you will be able to keep your patient's medical record complete.
